Form 8 decision tree
The Election Commission’s current Form 8 is for an already enrolled elector who seeks shifting of residence, correction of an existing entry, replacement of EPIC without correction, or marking as a person with disability. The form requires the applicant to choose the relevant option and complete the matching section.
- Your current entry exists and a particular is wrong: select correction of entries and identify the exact item to be corrected.
- You have shifted residence: use the shifting option and apply to the ERO for the constituency where the new address is located.
- Your name is not included at all: first confirm the current roll and constituency. Form 6, rather than a correction request, may be the appropriate inclusion route.
- A deletion objection or notice is pending: answer the notice and hearing requirements. Form 8 should not be treated as an automatic substitute for that proceeding.
Prepare one clear correction request
- Copy the current roll entry or EPIC details exactly as they appear.
- Write the requested corrected entry in one consistent form, including Bengali and English spellings where both matter.
- Attach the document that directly supports the particular being corrected and identify that document in the application.
- If different records use different spellings, add a short chronology or explanation instead of submitting unexplained inconsistencies.
